To attend DCON in Anaheim, Montgomery High School students need SEVEN things:
(1) Active dues paid membership in Montgomery High School Key Club; (2) First payment of $154 by January 15, 2017 (if by check, payable to Tricia Shindledecker), total student portion expected to be $200, with balance due January 22, 2017 [fundraising options available]; (3) Completed and signed Permission Form turned in by January 22, 2017; (4) Completed and signed (updated) Medical Info form turned in by January 22, 2017; (5) Code of Conduct Form turned in by January 22, 2017; (6) Transportation Form turned in by January 22, 2017, if not already completed; and (7) Photo Release Form please, if not already completed for earlier events. Key Club members may attend on a space available basis. Questions should be directed to the Kiwanis Advisor, Tricia Shindledecker, (707) 888-7446. Students will complete a separate school absence permission form, available from Faculty Advisor, Ted Seche, which must be signed by the parent/guardian and each teacher whose class the student will be absent on Thursday, April 6 or Friday, April, 7, 2017. With advance permission from parents and teachers, the student's absence at this leadership event will be excused. |
